The United States Bankruptcy Court for the Northern District of Iowa is pleased to provide fax service to bankruptcy attorneys. The clerk’s office will be able to fax all limited notices, orders, and copy requests.
Fax service allows attorneys to receive these documents substantially faster than by mail. It also reduces our costs for postage, printing, and envelopes. As with our other automation initiatives, it is our goal to provide you with the best possible service using available technology.
We will not fax high-volume notices that go to all creditors, such as the notice of commencement of case. These high volume notices will continue to be mailed through the Bankruptcy Noticing Center. This service does not permit you to file documents by fax. It only allows you to receive fax service of court orders and notices.
If you are a practicing bankruptcy attorney in our district and are interested in receiving court documents by fax, please complete and return the reply card. You will be responsible for notifying the clerk’s office in writing if your fax number changes or if your fax machine will be out of service for an extended period of time.
Matrix Requirements |

The matrix is a list of the names and addresses of creditors and parties in interest required
to receive notice in a bankruptcy case. It is scanned electronically into the court's computer for
noticing purposes. The debtor and attorney for debtor are responsible for complete information
on the matrix.
The matrix shall conform to the following requirements
- The matrix must be on standard 8 ½ x 11" white paper in single column
format. The matrix must be typed using either:
- Courier
- Arial
- Times New Roman
typeface. All characters and lines on the matrix must be properly aligned and justified at the left
margin. The type must be clear and readable. Keep punctuation and symbols to a minimum.
- Do not include the debtor, joint debtor, attorney for the debtor, or the Office of the U.S.
Trustee. They will be retrieved automatically from the debtor's petition.
- The matrix must be an original document separate from the petition with no stray marks,
stains, or titles on its face.
- Upper case only (all capital letters), bold, and/or italic typefaces are not acceptable.
Type in upper and lower case as you would on a letter.
- The matrix must be typed so that margins are no closer than one (1) inch from all edges of
the page.
- Each party's name/address block must consist of a minimum of three (3) lines of text and a
maximum of five (5) lines. Do not place a blank line within a name/address block.
- Each name/address block must be separated by at least three (3) blank line spaces. Each
entity must only be listed once.
- Each line of text may be no more than 40 characters long.
- The last line for each individual entity MUST contain each of the following items in the
following order: city, state, and zip code. If a nine-digit zip code is used, it must be typed
with a hyphen separating the two groups.
- All states must be two-letter (two characters only) abbreviated without spaces or periods.
Example: correct="DC"; wrong = "D.C." or "Dist. of Columbia".
- On the back of each page of the matrix, type the debtor's name, the page number,
and total number of pages in the matrix. Example: John Doe, Page 1 of 2.
- ALWAYS INCLUDE THE FOLLOWING PARTIES AS INDICATED (current
matrix list is maintained by the Clerk's Office):

Internal Revenue Service (Chapter 11 only)
Iowa Department of Human Services (If overpayment of public benefits such
as Aid to
Dependent Children, Food Stamps, Title XIX Medicaid, or child support assigned to the
Department exists)
REGARDING NOTICE TO BE GIVEN WHEN AN AGENCY OF THE UNITED STATES
IS A CREDITOR:
Any agency, department, or instrumentality of the United States that is a creditor of a debtor
shall be included in the lists, schedules, statements, and/or matrices of creditors using the most
current address available. If the debtor-creditor relationship is with a federal agency whose
office is located outside the state of Iowa, the out-of-state address should be used in addition to
the addresses contained in the list maintained by the Clerk's Office.

In addition to the specific agencies listed above, the matrices of creditors shall also include the
appropriate address of the United States Attorney for the Northern District of Iowa. The listing
of the United States Attorney must include a designation of the agency involved, using the
initials of that agency. For example, "United States Attorney (FSA)."
